Pagan River Madness displays a sludgy, doom-influenced classic metal sound. The song introduces itself with an ominous clean guitar riff and dark bass that introduces dynamic guitar chords. Vocals kick in and are in a melodic sung/spoken style with multiple other vocal tracks backing up the lead vocals. The song's chorus takes on a more malevolent sound and creates a dark yet catchy atmosphere.
Evil laughter throws the listener into the midst of soaring 2 and a half minute guitar solo that fades out the song with a sea of rapid guitar runs and riffage.
